The ventilator tube connecting a patient who is paralyzed and 100% ventilator dependent accidentally detached. An alarm went off to alert medical staff, but no one responded promptly. The ventilator tube remained detached for several minutes. The lack of oxygen put the patient into respiratory arrest, which ultimately escalated into cardiac arrest. If medical staff had responded to the alarm immediately or within the first few minutes of the ventilator tube disconnecting, all or most of the anoxic brain injury could have been prevented.
