Anterior Thoracolumbar Fusion Revision Surgery

The surgical steps of an anterior thoracolumbar fusion revision are depicted in this exhibit. First, a thoracotomy is made over the right ribs so a section of the 9th rib can be removed, which will later be used as a bone graft. The thoracic spine is accessed anteriorly, and residual disc material and bone chips are removed between the T11 and T12 vertebrae. Next, a partial corpectomy of T11 is performed, a discectomy is conducted, and the endplates are prepared at T10-11. Harms cages filed with autograft are placed in the T10-12 disc spaces. Finally, morselized bone graft is packed around the cages.
