This exhibit depicts a cervical discogram, discectomy, and fusion carried out at C4-5. A discogram is performed to confirm the presence of a disc herniation at C4-5. An incision is made over the anterior neck to expose the cervical vertebrae. A discectomy and bilateral foraminotomies are performed at C4-5 to decompress the spinal cord and nerve roots. A bone graft-filled interbody cage is inserted in the disc space and affixed with an anterior plate and screws.
